How Safety Management Systems change the value of a transcript

The FAA describes a Safety Management System as a formal, organization-wide approach to managing safety risk and ensuring safety controls are effective. Part 5 SMS has long applied to Part 121 carriers, and the FAA’s 2024 expansion brought additional Part 135 operators, certain Part 91.147 air-tour operators, and specified Part 21 design and production organizations into mandatory SMS implementation on phased timelines. 2. Preserve the original. Maintain the original file, metadata, time zone, device or system source, and any chain-of-custody information. Work from an authorized copy when appropriate.

3. Build a participant and terminology packet. Provide names and roles, call signs where approved, aircraft type, tail or fleet identifier where permissible, airport codes, route, equipment terms, maintenance acronyms, and known event times.

4. Choose transcript fidelity deliberately. Use full or near-verbatim for investigations and disputed events; use clean verbatim for training or safety-promotion content only when omissions and cleanup will not compromise meaning.

A useful transcript starts with a structured header: event or case number, date, time standard, aircraft or operation identifier, recording source, participants and roles, transcript status, verbatim level, timestamp method, source duration, and any known audio limitations. If the organization uses UTC, say so explicitly. If elapsed time is used, label it as elapsed time rather than allowing readers to mistake it for event time.

Speaker labels should reflect operational roles when that helps interpretation: CAPTAIN, FIRST OFFICER, DISPATCH, MAINTENANCE CONTROL, STATION OPS, CABIN LEAD, ATC, INVESTIGATOR, or WITNESS. Use names only when authorized and necessary. If call signs or employee numbers are sensitive, use controlled identifiers. Never infer a speaker’s identity from context when the audio does not support it; mark the uncertainty and escalate it for client review.

Why timestamps are more important in aviation than in ordinary meetings

Aviation investigations are timeline-driven.
